Scenario:
You are a nurse at Midtown Primary Care Clinic. Your patient has chronic back pain not responding to initial treatment.
Case Notes:
Patient: Mrs. Sonia Patel
Age: 52
Occupation: School Teacher
Visit Dates: 15 April, 30 April, 15 May 2025
Presenting Complaint:
• Lower back pain (8 months)
• Worse after prolonged sitting/standing
• Disturbs sleep occasionally
• No bowel/bladder symptoms
Medical History:
• Type 2 Diabetes – controlled
• Hypothyroidism
• Overweight (BMI 31)
Investigations:
• Lumbar X-ray: mild disc degeneration
• MRI suggested (not yet done)
Treatment to Date:
• Naproxen 500mg b.d. (some relief)
• Physiotherapy (4 sessions, minimal benefit)
• Advised postural exercises and weight loss
Social:
• Lives with husband, one teenage child
• No history of trauma
